A clinical scoring system for acute appendicitis

1. Migratory right iliac fossa pain

Pain begins in the periumbilical region and then migrates to the right iliac fossa (1 point)

2. Anorexia

Loss of appetite (1 point)

3. Nausea or vomiting

Nausea or vomiting present (1 point)

4. Tenderness in right iliac fossa

Tenderness on palpation in the right iliac fossa (2 points)

5. Rebound tenderness

Pain upon removal of pressure rather than application of pressure (1 point)

6. Elevated temperature

Oral temperature > 37.3°C (99.1°F) (1 point)

7. Leukocytosis

White blood cell count > 10,000/mm³ (2 points)

8. Shift to the left (neutrophilia >75%)

Neutrophils >75% of total WBC count (1 point)
